Technical details wanted

From the paragraph under the "Level Two" subhead, I infer some sort of print-on-demand system. What software was used for page layout and design? What sort of database file? What software or plug-in to merge the files? What kind of press? The paragraph on color refers to "toner" -- was this printed on a color laser printer? If so, how did they manage the 4/c bleed on the top edge?
