The qualification period for the prize drawings of the sweepstakes begins on November 16, 2009 and ends on December 6, 2009. To enter the contest, you must sign up to become a member of Creativepro.com or sign in as an existing member of Creativepro.com. Follow all instructions and complete the Creativepro.com online membership form.
No mechanically reproduced entries accepted. Sponsor not responsible for error, omission, interruption, deletion, defect, delay in operations or transmission, theft or destruction or unauthorized access to or alterations of entry materials, or for technical, network, telephone equipment, electronic, computer, hardware or software malfunctions of any kind, or inaccurate transmission of, or failure to receive entry information by sponsor on account of technical problems or traffic congestion on the Internet or at any web site or any combination thereof. Winners will be notified by electronic mail. Creativepro.com will attempt to re-send said email up to 2 times. If any winner does not respond to email notification within three days of its transmission, or should the email be returned as undeliverable after 2 attempts, the prize will be forfeited. Winners may be required to sign and return an Affidavit of eligibility/Liability/Publicity Release within 10 days of date printed on notification or s/he will be disqualified and an alternate will be selected at random. Winners may not substitute or transfer prizes. Sponsor reserves the right to substitute a prize of equal or greater value in the event a prize is unavailable. Prizes are not redeemable for cash. All taxes on prize are winners' responsibility. Limit: one prize per household. Except where prohibited by law, entry constitutes permission to use winners' names, hometowns and likenesses for online posting, and/or any advertising and publicity without additional compensation.
